This award recognises the growing importance of healthcare services being delivered ever closer to the patient, often in their own home. “Patient centricity” is a key principle of health policy in many countries. The reshaping of services which have been traditionally delivered in secondary care has the potential to deliver high quality health outcomes, and an improved patient experience in an increasingly cost-contained healthcare environment.
Entries to this category should ideally demonstrate the rationale for delivering the services in the community or in the home, should show the outcome produced, and the impact from the patient’s as well as the professional’s perspective.
