Positive Conversations

Summary of work

It is well recognised that higher quality communication and relationships are associated with improved patient engagement in HIV care and outcomes. This has been increasingly meaningful in recent times as patient visits are reduced in secondary care settings for patients with chronic manageable conditions. This coupled with reported patient reluctance to share other clinical and non-clinical concerns has resulted in missed opportunities by both parties to meaningfully engage. Positive Conversations was designed to support patients to identify and report issues they are experiencing in a safe environment at the point where action to resolve the issue can taken.

The initiative was developed by patient organisations, multidisciplinary healthcare teams and Gilead. Two simple, highly visual, reproducible and scalable tools were produced: Positive Conversations eBook and Wellness Thermometer, for use by both patients and HCPs in a variety of settings.

Following a pilot in three HIV clinics, quantitative patient and qualitative HCP research demonstrates that the Positive Conversations intervention led to improved patients’ identification and disclosure of their concerns prior to and during consultations, enhancing the quality of the conversations between patients and their care team.
